Job Title: Client Advocate
Classification: Non-Exempt (Full-time)
Job Reports To: Client Services Manager
Next Level: Chief Financial Officer
Job Summary:
Major responsibility is the completion of a face to face initial and yearly assessment on individuals in need of nutritional services and make appropriate referrals for all other services needed. Assist with clients in crisis or engage with emergency contacts as needed.
Major Responsibilities:
- Establish contact with individuals in need of services (and caregivers/families) to schedule face-to-face assessment within the parameters of client case management program.
- Drive to prospective clients’ home to complete an accurate assessment on all eligible clients and make the appropriate referrals to Aging and Disability Resource Centers (“ADRC”).
- Visit current clients face to face at a minimum of once per year to complete reassessment.
- Provide resources for older, homebound, isolated persons.
- Utilize our proprietary data management software and maintain HIPAA compliance.
- Perform clerical responsibilities as they relate to Client Services program.
- Cross-train in the functions of e-CIRTS reporting for the Client Services Manager
- Maintain a schedule of appointments and office time according to the master schedule as maintained by the Client Services Manager.
- May perform other duties as required by workload and supervisor.
Minimum Requirements:
- High School diploma or equivalent
- 2 years case management experience.
- Valid drivers’ license with a reliable automobile.
- Must be at least 23 years old and have a good driving record to meet Meals on Wheels PLUS of Manatee’s automobile insurance requirements.
Skills, Tools, and Technology Requirements:
- Ability to work independently and interact with elderly and disabled people while gathering confidential information with discretion and compassion.
- Ability to perform data entry with complete accuracy.
- Ability to follow directions.
- Must be detail oriented.
- Must be able to demonstrate excellent computer and telephone skills.
- Ability to assess the social and economic needs of individuals and accurately document these findings.
- Ability to obtain certification in UCAT training applicable to the position requirements.
- Must have very good oral and written communication skills and be able to follow supervisory directions.
- Must be able to bend, lift, carry, push and pull 25 lbs. or 50 lbs. with assistance.
- Able to sit and stand for long periods of time.
- Bi-lingual preferred.
Meals on Wheels Plus is a “Drug Free" workplace. Drug testing is mandatory, with a required “negative" result for all federally recognized drugs, including medically prescribed cannabis.
