Job Summary: A Certified Home Health Aide (CHHA) is responsible for providing assistance and personal care services to elderly, disabled, or chronically ill individuals in their homes. The main goal is to help clients maintain their independence and improve their quality of life while ensuring their health and safety. Responsibilities: 1. Assist clients with activities of daily living (ADLs) such as personal hygiene, dressing, grooming, and bathing. 2. Aid and support in mobilization, including transferring, lifting, and positioning clients as required. 3. Provide assistance with meal preparation, feeding, and medication reminders. 4. Help in maintaining a clean and organized living environment for clients. 5. Observe and report any changes in the client's condition or behaviors to the appropriate healthcare professionals. 6. Follow care plans, including therapeutic and supportive measures as directed by the supervising registered nurse or licensed healthcare professional. 7. Provide emotional support and companionship to clients. 8. Document services provided accurately and timely. 9. Maintain confidentiality of client information and respect their privacy. 10. Adhere to all agency policies, procedures, and safety guidelines. 11. Report changes observed in clients to supervising RN, such as: falls, changes in mental status, hospitalizations, ect. 12. Provide assistance with Instrumental activities of daily living (IADL’s) such as transportation, errands, light housekeeping, and/or meal preparation and planning. Requirements: 1. Must be a certified Home Health Aide in the state of New Jersey. 2. High school diploma or equivalent. 3. Valid driver's license and reliable transportation. 4. Clear criminal background check and drug screening. 5. Ability to communicate effectively in English, both verbal and written. 6. Compassionate, patient, and understanding towards clients, treating them with dignity and respect. 7. Good time management and organizational skills. 8. Demonstrated ability to work independently as well as in a team. 9. Basic computer skills for documenting care provided. Working Conditions: 1. CHHAs in NJ typically work in clients' homes, which may involve traveling to different locations. 2. May involve working weekends, evenings, or holidays, depending on client needs and schedules. 3. Physical demands include standing, bending, lifting, and assisting with transfers. 4. Potential exposure to communicable diseases or hazardous materials, requiring adherence to infection control protocols. 5. Must be able to lift 25 pounds.
