Salary : The Role :

As a Community Health Worker (CHW) in the Maternal Health Program , you will play a vital role in supporting our mission to improve maternal and infant health outcomes. In this role, you will provide direct support, education, and advocacy for high-risk pregnant women and new mothers throughout pregnancy, childbirth, and the postpartum period. You will serve as a trusted community resource, helping members navigate healthcare and social services to ensure they receive the support they need.

What Youll Do :

  • Build relationships with pregnant women and new mothers in the community, providing guidance, education, and emotional support.
  • Conduct home visits and community-based check-ins to assess member needs and connect them with essential healthcare and social services.
  • Conduct intake assessments to identify members' medical, social, and logistical needs.
  • Provide culturally competent health education on prenatal care, childbirth preparation, postpartum recovery, infant care, and family wellness .
  • Assist members in accessing healthcare services, including prenatal and postpartum care, by helping with appointment scheduling and transportation coordination.
  • Connect members with resources such as nutrition programs, housing support, mental health services, and substance use treatment when needed.
  • Advocate for members by serving as a liaison between them and healthcare providers, ensuring they receive timely and effective care.
  • Support members in setting and achieving personal and family health goals, empowering them to take charge of their well-being.
  • Stay informed about best practices in maternal and infant health, community health work, and health disparities to enhance service delivery.
  • Maintain accurate and up-to-date records of member interactions, assessments, and referrals using electronic health record systems.

The Ideal Candidate :

  • Required : High school diploma or GED required; Associates or Bachelors degree in a health-related field preferred .
  • Community Health Worker Certification or willingness to obtain certification within before start date.
  • Experience in community outreach, case management, public health, or social services is highly desirable.
  • Familiarity with maternal and infant health, as well as the unique needs of high-risk and underserved populations.
  • Strong organizational and communication skills with the ability to manage a caseload effectively.
  • Passion for advocating for underserved communities and reducing health disparities.
  • Comfortable working in the field, conducting home visits, and meeting members where they are .
  • Proficiency in using electronic health record systems or willingness to learn.
  • Bilingual (Spanish / English) candidates are highly encouraged to apply .
  • Willingness to travel locally, especially to rural or underserved areas.
