Salary : The Role :

As a Delegated Care Coordinator in the Maternal Health Program, you will play a vital role in supporting our mission to improve maternal and infant health outcomes. In this role, you will coordinate care and provide comprehensive support to high-risk pregnant women and new mothers throughout pregnancy, childbirth, and the postpartum period. You will serve as an advocate, resource, and navigator, ensuring our members receive timely, effective, and culturally sensitive care during this critical time.

What Youll Do :

  • Conduct home visits periodically
  • Conduct thorough intake assessments to identify members' medical, social, and logistical needs.
  • Develop and maintain individualized care plans that align with members' health goals, provider recommendations, and available resources.
  • Serve as a liaison between members and healthcare providers to ensure continuity of care and timely access to necessary services.
  • Provide education and guidance on maternal and infant health topics, including prenatal care, childbirth preparation, postpartum recovery, and newborn care.
  • Coordinate referrals to counseling agencies, social services, community resources, and other programs to address barriers to care (e.g., food security, transportation, housing).
  • Regularly follow up with members to monitor progress, address concerns, and adjust care plans as needed.
  • Conduct home visits or virtual check-ins to assess living conditions, provide moral support, and ensure the well-being of members and their families.
  • Stay informed about best practices in maternal health, infant development, and care coordination to enhance service delivery.
  • Document and maintain accurate, up-to-date records of member interactions, assessments, and outcomes using electronic health record systems.

The Ideal Candidate :

  • Required : Bachelors Degree, or 2 years' experience in a related field (case management, social work)
  • Familiarity with maternal and infant health, as well as the unique needs of high-risk populations.
  • Strong organizational skills with the ability to manage a caseload of 20-25 members effectively.
  • Excellent communication and advocacy skills, with a commitment to cultural sensitivity and competence.
  • Ability to work collaboratively in a multidisciplinary team and independently prioritize tasks.
  • Proficiency in using electronic health recordkeeping systems or willingness to learn.
  • Willingness to travel for home visits, especially in rural or underserved areas.
  • Passion for improving maternal and infant health outcomes, particularly for underserved communities.
