Position Summary
Serves as a care coordinator, system navigator, and patient advocate with a mission to improve the care experience for cancer patients. Provides Social Work services in accordance with professional standards and established hospital policies and procedures. Services focus on enhancing the patient’s adjustment to illness and initiating referrals for continued care post discharge. Social Work Services reflect expertise in the management of multi-systems to ensure quality of service. Intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Offers short term therapeutic counseling and crisis intervention to patients and families with psycho-social needs.

Minimum Qualifications
- Previous experience in social work preferred.
- Previous experience working with oncology patients strongly preferred.
- Bachelor's degree in Social Services required. Master's degree preferred.
