Shift : Monday to Friday, 20 hours per week, no on-call or weekends
Start Date : As soon as credentialed
End Date : 6-month assignment with the possibility to extend
Position Details :
Patient Population : Frail and elderly
Setting : Community-based care
Responsibilities :
- Provide primary medical care and conduct assessments.
- Collaborate on care plans and discharge planning.
- Evaluate and treat acute illnesses.
- Conduct nursing home or home visits as required.
- Assist in developing clinical policies and provide staff guidance.
Days Per Month : 20 hours per week
On-Call / Weekends : None
Submission Requirements :
Master s Degree in Nursing - Required
5 years of experience in long-term care - Required
Active Pennsylvania license - Required
BLS, CDS, DEA certifications - Required
