RN Care Manager

Case management is a collaborative and rewarding process as you help others navigate through the complex and often overwhelming healthcare system; it can be a source of great personal satisfaction. We are seeking a compassionate RN, with top-notch communication, problem solving and time management skills for a full-time, direct hire opening!

Managing a caseload of approximately 21-25 members, you'll monitor the health and well-being of pregnant women and their children after birth. Your responsibilities will include providing interactive parent-child education focused on nurturing emotional connections and using motivational interviewing to empower clients. This proactive approach helps clients clarify goals, develop problem-solving skills, and achieve greater self-sufficiency. You'll build deep, meaningful relationships with clients through weekly or bi-weekly visits starting early in pregnancy until the child reaches two years old.

Minimum Qualifications:

• A valid NYS RN license in good standing is required
• BSN required, new graduates will be considered
• Prior case management, pediatric or Mother-Baby experience is preferred, but not required
