About the Facility : Our client, a well-established healthcare provider in Hempstead, is looking for a Spanish-speaking Nurse Practitioner (NP) to join their team for virtual establishment of care visits for home care patients. This role will involve working in the Hempstead facility while conducting virtual visits to establish care and provide follow-up care to patients.
Position Overview : As an NP, you will conduct virtual visits for home care patients, assessing their medical needs, establishing care plans, and ensuring the continuation of proper treatment. Fluency in Spanish is required for effective communication with the clinic's patient base.
Key Responsibilities :
