Responsibilities
- On-Going Patient/Family Education
- Initial/On-Going Patient Assessment
- Health Management
- Medication Management
- Chronic and Acute Disease Management
- Interdisciplinary Collaboration with Care Teams
- Management of Home Health Plan of Care
- Management of Skilled Goals and Interventions
- Direct Patient Care/Nursing Skills: Medication management, education and reconciliation; Wound care (basic and advanced, NPTW, etc); Drain care; Cath care; Ostomy care; VAD management and VAD site care; Venipuncture and blood sample collection via VAD; etc.
Qualifications:
- Must have current LPN License in good standing in the State of Ohio
- Valid driver’s license
- Be able to pass a criminal background check
Job Type: Part-time
Pay: $20.00 - $30.00 per hour
