Medadventures is seeking a dedicated Home Health Physical Therapist to join our team. This role involves evaluating and treating patients in their homes to help them regain mobility, improve strength, and enhance quality of life.

Responsibilities :

Perform comprehensive physical therapy evaluations in a home setting.

Develop and implement individualized treatment plans to meet patient goals.

Collaborate with an interdisciplinary team to ensure high-quality care.

Document patient progress accurately and timely.

Educate patients and families on treatment techniques and care plans.

Requirements :

Active Physical Therapist license in Connecticut.

Previous home health experience preferred but not required.

Strong communication and organizational skills.

Ability to work independently and manage a flexible schedule.

Reliable transportation.
