Job Description :
We are seeking an experienced Registered Nurse (RN) to join our Home Health team in McCall, ID. This role involves providing home health and hospice care to patients in their homes, including those located within a 90-minute radius from McCall. The position requires flexibility, including on-call shifts, especially during weekends and holidays.
Key Responsibilities :
- Provide in-home health and hospice care to patients, including assessment, medication management, and patient education.
- Coordinate with the interdisciplinary team to ensure high-quality care and continuity of treatment.
- Utilize the EpicCare Inpatient EMR system for documenting patient care and updates.
- Manage patient care with a focus on safety, comfort, and quality of life, especially for hospice patients.
- Rotate on-call weekend shifts and provide care as needed during off-hours.
- Holiday coverage may involve being on-call, with Thanksgiving being a priority for coverage.
