Home Health RN

Overview:

The Home Health Registered Nurse provides in-home care for patients, ensuring high quality nursing care while promoting independence and patient education. The RN plays a key role in assessing patients' needs, developing individualized care plans, coordinating with the healthcare team, and monitoring patient progress. This position requires strong communication skills, clinical judgment, and a compassionate approach to patient care.

Minimum Qualifications

• Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited program.

• Current Registered Nurse (RN) license in the state.

• Minimum of 2 years of clinical nursing experience, with at least 1 year in home health or a related field.

• Strong clinical assessment and patient care skills.

• Ability to work independently and manage time effectively.

• Proficient in electronic health records (EHR) systems.

• Excellent communication and interpersonal skills.

• Current CPR certification.

Job Responsibilities

• Conduct initial and ongoing assessments of patient health conditions.

• Develop individualized care plans in collaboration with patients, families, and healthcare providers.

• Administer medications and treatments according to physician orders and best practices.

• Educate patients and families on disease management, medications, and care techniques.

• Monitor and document patient progress and adjust care plans as needed.

• Coordinate with physicians, therapists, and other healthcare providers for comprehensive patient care.

• Supervise and provide guidance to Home Health Aides and LPNs.

• Ensure compliance with home health regulations and company policies.

• Assist with discharge planning and provide follow-up care coordination.

• Maintain accurate and timely documentation in electronic health records.

• Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.

• Perform any additional duties as assigned to meet the needs of the department and organization.

Physical Requirements

• Manual Dexterity: Ability to use a computer, medical equipment, and perform nursing tasks for extended periods.

• Mobility: Ability to move between patients' homes, including bending, standing, and assisting patients.

• Lifting: Ability to lift or move objects and patients, typically up to 50 lbs.

Reports to: Director of Clinical Services

• Supervises: Home Health Aides, Licensed Practical Nurses (LPNs)

• Internal Relationships: Works closely with:

• Nursing team (for patient care planning and coordination)

• Therapy team (for collaborative patient care)

• Care management team (for discharge planning and coordination) External Relationships: Liaises with:

• Patients and their families (for patient education and care management)

• Physicians (for care plan updates and medical orders)
