Minimum Qualifications
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) from an accredited program.
- Current Registered Nurse (RN) license in the state.
- Minimum of 2 years of clinical nursing experience, with at least 1 year in home health or a related field.
- Strong clinical assessment and patient care skills.
- Ability to work independently and manage time effectively.
- Proficient in electronic health records (EHR) systems.
- Excellent communication and interpersonal skills.
- Current CPR certification.
Job Responsibilities
- Conduct initial and ongoing assessments of patient health conditions.
- Develop individualized care plans in collaboration with patients, families, and healthcare providers.
- Administer medications and treatments according to physician orders and best practices.
- Educate patients and families on disease management, medications, and care techniques.
- Monitor and document patient progress and adjust care plans as needed.
- Coordinate with physicians, therapists, and other healthcare providers for comprehensive patient care.
- Supervise and provide guidance to Home Health Aides and LPNs.
- Ensure compliance with home health regulations and company policies.
- Assist with discharge planning and provide follow-up care coordination.
- Maintain accurate and timely documentation in electronic health records.
- Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.
- Perform any additional duties as assigned to meet the needs of the department and organization.
Physical Requirements
- Manual Dexterity: Ability to use a computer, medical equipment, and perform nursing tasks for extended periods.
- Mobility: Ability to move between patients' homes, including bending, standing, and assisting patients.
