Who You Are
You are a compassionate and driven Nurse Practitioner, licensed in Michigan, with at least one year of experience working with the frail or elderly population. What You'll Do
As a Nurse Practitioner, you will:
Provide comprehensive primary care to elderly patients, managing acute and chronic conditions while focusing on preventive care.
Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans that address medical, social, and behavioral health needs.
Conduct initial patient assessments, routine follow-ups, and acute visits as needed.
Work closely with physicians and other healthcare professionals to ensure seamless patient care.
Participate in utilization management meetings and uphold clinical documentation standards.
Educate patients and their families on disease management, treatment options, and preventive care strategies.
Engage in quality improvement initiatives and contribute to best practices for patient outcomes.
