Family/Adult Nurse Practitioner

We are looking for a long-term and full-time Family/Adult/Geriatric Nurse Practitioner to add to our team! MedComplete is a group of caring and hard-working nurse practitioners with over 600 patients. Due to our constantly expanding patient load, we are searching for a qualified and experienced Nurse Practitioner passionate about improving long-term quality of health. Our team travels to patients' homes and assisted-living facilities to treat patients who are unable to get to the doctor on their own. As well as see patients at our Katy clinic. MedComplete covers Houston and many surrounding areas. We are hoping to add a communicative and detail-oriented person to our staff who can work well in a fast-paced medical practice. The practice is rapidly expanding and there will be opportunities for growth for the right applicant.

Job Responsibilities, including, but not limited to:

• Assess physical and psychosocial status of assigned patients
• Prepares and Provides accurate documentation of the visit in a timely manner
• Provides medical care as needed and with the scope of practice of the Nurse Practitioner
• Actively participates in the coordination of the patient's needs including but not limited to medical supplies, medication refills, specialist referrals, home health referrals, and hospice evaluation in a timely manner
• Order and interpret diagnostic and therapeutic tests relative to patient needs in a timely manner
• Accurately completes medical necessity forms in a timely manner
• Consistently identifies needs and gaps in care and initiates appropriate referrals to health plans, improvement activities, and individualized health care plans
• Able to drive up to a 60 mile radius for home or facility visits
• Able to perform acute visits for assigned patients

Qualifications

• Graduate of an accredited Nurse Practitioner Program and certification approved or recognized by the State of Texas DEA license (required)
• Qualified candidates must have a minimum of 2 years experience within the past 5 years
• 1 year Experience with EMR systems (preferred)
• Must be able to work full-time (day shifts)
• Able to drive a 60 mile radius for home/facility visits.
• Should be organized and detail-oriented and work well in a fast-paced setting.
• Most important is a positive attitude and a passion to provide quality care
