Position Details :

We are seeking a dedicated and experienced Pod Leader to oversee a team of Licensed Practical Nurses (LPNs) responsible for reviewing patient charts and managing patient care. The Pod Leader will play a crucial role in ensuring the accuracy and completeness of patient records, enhancing patient outcomes through effective case management, and fostering a collaborative environment within the healthcare team.

Key Responsibilities :

• Team Management : Supervise, mentor, and support a team of LPNs. Provide guidance and leadership to ensure high-quality patient care and accurate documentation. Provide education for assigned care coordinators when necessary.
• Patient Care Coordination : Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Case Management : Monitor and evaluate patient progress, making necessary adjustments to care plans. Collaborate with interdisciplinary teams to optimize patient outcomes and address any gaps in care.
• Documentation Oversight : Ensure the accuracy and completeness of patient electronic medical records (EMR). Review patient records to confirm that they reflect the conditions and treatment requirements of each patient.
• Reporting and Improvement : Complete and review reports to identify and address patient care gaps and reduce hospitalizations. Implement strategies to enhance care delivery and patient satisfaction.

Qualifications :

Salary Range : $85,000.00 per year
