Position Details :

We are seeking a friendly and compassionate travel nurse practitioner with experience in Endocrinology to join our team and support patients in the Patient Health Management (PHM) program. In this role, you will work closely with care coordinators across various providers and settings to promote improved health outcomes. You will facilitate patient assessments, assist with communication between facilities and patients, and ensure accurate documentation through our proprietary software system. You will receive regular support and guidance under the supervision of an offsite Collaborative Physician. Responsibilities :

• Assist care coordinators across the different providers and settings
• Promote better health outcomes through communication between facilities and patients
• Assess patients with endo conditions and reporting through our proprietary software system
• You will be cross-trained in other areas of care management
• Work under the supervision of an offsite Collaborative Physician
• Gathering and analyzing patient information
• Detailed review of the system
• Review of all pertinent patient labs, studies, and provider notes

Requirements :

Salary Range : $150,000.00 - $200,000.00
