- Collaborate with physicians, therapists, and other healthcare team members to ensure seamless care delivery.
Responsibilities:
- Conduct comprehensive assessments to identify patient needs and develop effective care plans.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care techniques.
- Coordinate care with other healthcare professionals, including physical therapy, occupational therapy, and speech therapy.
- Document patient progress and outcomes accurately and thoroughly, ensuring compliance with regulatory requirements.
- Maintain accurate and up-to-date records of patient information, including medical history, medications, and test results.
Requirements:
- Certified as a Registered Nurse (RN) in the state where you practice.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
