Full-time Home Health Registered Nurse Position
We are seeking a dedicated and compassionate Registered Nurse to provide high-quality patient care in their homes.
Responsibilities:
- Coordinate patient care with other healthcare professionals, ensuring continuity of care by planning and exchanging information with the physician, agency personnel, patient, family, and community resources.
- Conduct initial and comprehensive nursing evaluation visits to determine the primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders.
- Perform patient comprehensive assessments at designated time points and develop an appropriate plan of care.
- Verify that patients meet home health eligibility and medical necessity guidelines as defined by payer source.
Requirements:
- Current RN licensure in the state of practice.
- Current CPR certification.
- Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
