Mederi Caretenders is seeking a dedicated and experienced Home Health Clinical Supervisor to join our team. In this role, you will be responsible for overseeing the clinical operations of our home health team.
You will work closely with an interdisciplinary team of staff to ensure the continuity of high-quality care to home health patients assigned to your area. This includes managing referrals, ensuring timely evaluations by clinicians and therapists, and coordinating patient needs assessments and care plans.
- Key Responsibilities
- Manage referrals and ensure appropriate clinician assignments for timely patient evaluations.
- Coordinate determination of patient home health benefits and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs.
- Oversee development, implementation, and updates to the individualized patient plan of care.
- Document phone calls from physicians, clinicians, patients, and referral sources.
- Review daily assessments and plans of care and consult with clinicians as needed.
- Coordinate all aspects of care with disciplines, physicians, durable medical equipment providers, caregivers, transferring facilities, and other healthcare providers.
- Follow-up on lab results, physician contact, and significant changes in patient condition.
- Schedule, facilitate, and document case conferences and SOC reports.
- Assist clinicians in coordinating patient transfers and discharges.
- Process new orders and update visit frequencies as needed.
- Write and process orders when taking verbal orders directly from physicians.
- Assure payer change documentation is completed properly and timely.
- Review clinician visit notes weekly to ensure timely submission of all documentation.
- Review service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies.
- Enter infections and incidents into the online Risk Management Incident Reporting System.
- Assist in the orientation of new agency personnel.
- Provide direction and leadership to clinical team members.
- Participate in QAPI program.
- Direct clinicians in utilizing best practice interventions when finalizing Plans of Care.
- Participate in on-call rotation.
