- The RN coordinates the total plan of care with other healthcare professionals involved in patient care, ensuring continuity and effectiveness. This involves exchanging information with physicians, agency personnel, patients, families, and community resources within a Point of Care setting.
- The RN makes initial and comprehensive nursing evaluations, determines the primary focus of care, develops plans within state-specific guidelines, and submits accurate and timely documentation in collaboration with physician orders.
- The RN performs patient comprehensive assessments at designated time points and develops appropriate care plans.
