What are the responsibilities and job description for the Care Manager (RN) (Corpus Christi Only) position at Media Riders Inc.?
*** RN Certified Required ***
** 4-6 years of experience working with the adult Aged, Blind, or Disabled population. **
** Bilingual in Spanish and English would be a HUGE PLUS, but not a must have**
** RUG Certification would be a HUGE PLUS, but not a must have**
Day to day responsibilities of this the role and a description of the project:
- Identify and prioritize tasks that are urgent/due.
- Contact members and providers to check on status of services/items/healthcare needs.
- Document all contacts within the member’s file, dealing with majorly the Pediatric population.
- Complete in-home assessments (Visit homes 3 - 4 times a week) Traveling up to the 100 mile radius area of the city this role is assigned in is a requirement of this role.
Performance and Expectations / Metrics for this individual and their team:
- Completion of contractual telephonic and face to face assessments per member/LAR request on a monthly basis
- Ensure members services and DME items are in place
- meet audit score of 95%
Salary : $48 - $52
