The Clinical Pharmacist will support pharmacy quality initiatives to optimize healthcare outcomes. This role will provide a variety of Medication Therapy Management (MTM) services including proactive medication adherence outreach, patient education, and comprehensive medication reviews. The full salary range for this position is $98,400 - $168,600. Annual salary range placement will depend on a variety of factors including, but not limited to, education, work experience, applicable certifications and/or licensure, the position's scope and responsibility, internal pay equity and external market salary data. In addition to base compensation, this position may be eligible for incentive plan compensation in addition to base salary. Key Accountabilities
- Conduct Comprehensive Medication Reviews (CMRs) to assess medication efficacy, safety, and adherence.
- Proactively engage patients to address barriers to medication adherence.
- Identify and resolve medication-related problems, including drug interactions, therapy duplications, and cost-saving opportunities.
- Provide patient education on proper medication use, potential side effects, and lifestyle modifications to improve health outcomes.
- Collaborate with prescribers and pharmacies to recommend medication adjustments.
Qualifications
- Bachelor’s degree in Pharmacy required; PharmD preferred
- 5 years of related clinical experience
- Valid, registered pharmacy license (in any state)
- Prior experience with Medicare Stars is preferred
- Experience working in a matrixed or multi-disciplinary environment is preferred
- Excellent communication and patient counseling skills
- Organized individual who can simultaneously balance and execute through multiple projects and priorities
- Self-starter, possessing willingness and ambition to work through ambiguity
