Overview
The Licensed Practical Nurse is responsible for providing direct or indirect nursing care to patients who present with urgent, emergent, or potential health problems; utilizes knowledge of basic nursing principles, practices and ethics; participates in the organization and management of the physicians’ practice.

Responsibilities
- Ensures pre-visit planning workflow is completed prior to visit, whenever possible
- Optimizes care coordination with; hospitals, emergency room, consulting physicians, and community resources as needed
- Develops workflow to ensure smooth transition of care for patients treated in facility by a specialty position or other healthcare provider
- Reviews after visit summary with patients when appropriate
- Triage patients to ensure productive departmental flow
- Obtaining pre-authorizations as needed
Qualifications
- Graduation certificate from accredited LPN or RN school of nursing
- Active LPN or RN License
- 2 years’ experience preferred, but not required
