Responsibilities
- Conduct thorough assessments of patients with acute and chronic wounds.
- Develop individualized treatment plans based on patient needs and evidence-based practices.
- Perform wound care procedures, including dressing changes, debridement, and advanced wound therapies.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
- Educate patients and their families on wound care management and prevention strategies.
- Monitor patient progress and adjust treatment plans as necessary.
- Maintain accurate documentation of patient assessments, treatments, and outcomes.
- Participate in case management activities to ensure continuity of care.
- Stay current with advancements in wound care techniques and technologies.
Requirements
- Master’s degree in Nursing with specialization as a Nurse Practitioner.
- Current Nurse Practitioner certification and state licensure to practice.
- Experience in wound care or related fields is preferred.
- Strong skills in case management and patient education.
- Basic Life Support (BLS) certification required.
- Familiarity with SNF procedures and practices.
