Home Therapy RN - Santa Rosa
We are seeking a dedicated Home Therapy RN to initiate patient care coordination and collaborate with the care team to provide exceptional care for individuals with kidney disease. In this role, you will conduct home visits, assess the suitability of home dialysis setups, and promote patient independence. You'll also have the opportunity for professional growth and contribute to improving the lives of patients.
Requirements :
- 1 year of RN experience, with at least 1 year in nephrology (peritoneal dialysis, home hemodialysis, or both)
- Graduation from an accredited nursing school
- Active RN licensure and CPR certification
- Strong knowledge of Electronic Health Records
- Valid driver's license and reliable transportation
