Job Title: RN Case Manager (Contract)
Location: Lakewood CO USA
Schedule: Monday–Friday | 8:00 AM – 5:00 PM with on call rotation
Contract Duration: 04/04/2025 to 06/26/2025
Pay Rate: $57-$60/hour
Job Type: Full-Time Contract

Overview:
We are seeking an experienced RN Case Manager to perform comprehensive assessments for high-risk members and develop individualized care plans that optimize outcomes and resource utilization. You will coordinate care across providers, manage transitions, and ensure non-medical support like housing and transportation is in place to support compliance and recovery.

Responsibilities:

• Perform comprehensive assessments of high-risk members
• Develop and implement individualized care plans in collaboration with PCPs
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
• Reassess care needs and ensure cost-efficient, appropriate levels of care
• Address non-medical needs such as housing and transportation
• Engage specialty resources to achieve optimal outcomes
• Maintain detailed documentation of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Prevent or minimize adverse patient outcomes through proactive intervention
• Serve as a patient advocate, maintaining privacy and confidentiality

Qualifications:

• Associate’s or Bachelor’s Degree in Nursing or a related field
• Current, unrestricted state RN license
• Case Management certification preferred
• Strong clinical judgment and decision-making skills
• Ability to analyze complex medical data and coordinate across multiple disciplines
