Job Description
Medi Home Health in Roanoke, VA, a division of Medical Services of America, Inc., currently seeks a Clinical Manager, RN for our Home Health Team that supports patients in Roanoke, VA (Roanoke, Vinton, Botetourt, Bedford, Salem, and Rocky Mount)
- Provides daily direct clinical oversight, supervision and team management of the interdisciplinary team.
- Serves as the clinical liaison between referral sources, staffing and clients.
- Review patient referrals and assure complete intake process prior to patient start of care.
- Monitor daily clinical operations, quality assurance and patient care outcomes.
- Monitor outcome measures and performance tools for enhance patient care.
- Provides ongoing necessary education and training to the team and conducts weekly / monthly staff meetings.
- Provide direct patient care as applicable
- Works with staff on scheduling and assignments for optimal efficiency.
- Regularly reevaluates staff competencies and need for continued education and update of policies and procedures in working with administrator.
- Direct the interdisciplinary team on initiating diagnostic, preventive and rehabilitative procedures as appropriate to the client’s care and safety as needed.
- Teaches, supervises and counsels the client and family regarding home nursing procedures and other care needs as appropriate to the client’s condition as needed.
Qualifications And Skills
- Completion of a nursing program from an accredited school or university required.
- Must be currently licensed as an RN in the state of VA.
- Minimum of two-year home health nursing experience preferred, minimum of three - year nursing required.
- Minimum of one-year supervisory experience preferred.
- Home Health experience strongly preferred.
- Valid Driver's License required.
