About the Role

Responsibilities
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with registered nurses and physicians.
- Assess patient needs and update care plans as necessary.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
Requirements
- Degree from an accredited LPN program.
- Current licensure as an LPN in the state of NC.
- Minimum one year of experience in a home health setting.
- Ability to lift 25 pounds and stand for long periods.
