Key Responsibilities:
Assessing patients’ healthcare needs and developing individual care plans.
Coordinating patient care across multiple disciplines to ensure continuity and quality of care.
Collaborating with physicians, nurses, and other healthcare professionals.
Monitoring patient progress and adjusting care plans as needed.
Advocating for patients and their families to ensure they receive the services they need.
Discharge planning: Developing and coordinating effective discharge plans to ensure a smooth transition from care.
Ensuring patients and families are informed about post-care instructions and necessary follow-up services.
Qualifications:
Must be a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Social Worker.
Previous experience in case management or a healthcare setting preferred.
Strong communication and organizational skills.
Ability to work independently and as part of a team.
