About the Role:

The RN-UM Case Manager plays a critical role in ensuring that patients receive appropriate and effective care throughout their healthcare journey. This position involves assessing patient needs, coordinating care plans, and collaborating with healthcare providers to optimize patient outcomes. The RN-UM Case Manager will also be responsible for monitoring the utilization of healthcare resources to ensure compliance with established guidelines and regulations. By advocating for patients and facilitating communication among all stakeholders, this role aims to enhance the quality of care while managing costs effectively. Ultimately, the RN-UM Case Manager contributes to the overall mission of improving patient health and satisfaction within the non-durable goods manufacturing industry.

Minimum Qualifications:

  • Current Registered Nurse (RN) license in the state of Alabama.
  • Associates or Bachelor's degree in Nursing (BSN)
  • Minimum of 3-5 years of clinical nursing experience, preferably in case management or a related area.
  • BLS certified or must obtain within 30 days of hire.

Preferred Qualifications:

  • 2 years of leadership experience.
  • Certification in Case Management (CCM) or Utilization Management (UM).
  • Experience in a non-durable goods manufacturing environment or similar healthcare setting.
  • Knowledge of healthcare regulations and compliance standards.

Responsibilities:

  • Conduct comprehensive assessments of patients' medical, psychological, and social needs to develop individualized care plans.
  • Coordinate with healthcare providers, patients, and families to ensure seamless transitions of care and effective communication.
  • Monitor patient progress and adjust care plans as necessary to meet changing needs and improve health outcomes.
  • Evaluate the utilization of healthcare services and resources, ensuring compliance with regulatory standards and organizational policies.
  • Provide education and support to patients and families regarding treatment options, resources, and self-management strategies.

The required skills for this position include strong clinical assessment abilities, which are essential for evaluating patient needs and developing effective care plans. Excellent communication skills are vital for collaborating with healthcare teams and educating patients and families about their care. Organizational skills are necessary to manage multiple cases and ensure timely follow-up on patient progress. Critical thinking skills are employed daily to analyze complex situations and make informed decisions regarding patient care. Preferred skills, such as knowledge of healthcare regulations, enhance the ability to navigate compliance issues and optimize resource utilization.
