About the Role:
We are seeking a highly motivated and compassionate Social Worker-MSW to join our team in Case Management Department. As a Social Worker-MSW, you will be responsible for providing support and counseling to patients and their families at UAB Medical West. Your primary goal will be to improve the quality of life for patients by addressing their emotional, social, and financial needs. You will work closely with other healthcare professionals to develop and implement treatment plans that meet the unique needs of each patient.
Minimum Qualifications:
- Master's degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Alabama.
- LGSW required.
- Experience working in a healthcare setting, preferably in an acute care hospital.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Preferred Qualifications:
- Minimum of six months acute care experience preferred.
- LCSW is a plus.
- Experience working with diverse patient populations, including those with chronic illnesses and disabilities.
- Knowledge of community resources and services available to patients and their families.
- Experience providing crisis intervention and support.
Responsibilities:
- Conduct psychosocial assessments of patients and their families to identify their needs and develop appropriate treatment plans.
- Provide counseling and support to patients and their families to help them cope with the emotional and social challenges of illness and hospitalization.
- Collaborate with healthcare professionals to develop and implement treatment plans that address the physical, emotional, and social needs of patients.
- Advocate for patients and their families to ensure that their rights and needs are respected and addressed.
- Maintain accurate and up-to-date records of patient assessments, treatment plans, and progress.
Skills:
As a Social Worker-MSW, you will use your strong communication and interpersonal skills to build relationships with patients and their families. You will also use your knowledge of community resources and services to connect patients with the support they need.
