Job Title: Physician - Family Practice/Internal Medicine

Position Overview:
We are seeking a compassionate and dedicated Family Practice or Internal Medicine Physician to join our innovative and patient-centered team. This role offers the unique opportunity to practice within a value-based care model, with a primary focus on diabetes care and the promotion of lifestyle medicine. Caring for a unique and rewarding patient population of First Responders, you will have a direct impact on improving patient health outcomes by addressing not only clinical management but also the prevention of chronic disease through personalized lifestyle interventions. This is a full-time, 36-hour a week position, Monday - Friday, which requires traveling between 4 clinic locations (set schedule). No call and no after-hours charting. Key Responsibilities:

Provide comprehensive, patient-centered care to individuals with diabetes, managing both the clinical aspects of the condition and its associated comorbidities.

Develop individualized care plans that emphasize sustainable lifestyle changes, including nutrition, physical activity, stress management, and behavioral health.

Collaborate closely with a multidisciplinary team, including dietitians, diabetes educators, clinical pharmacists, and care coordinators, to drive optimal outcomes and ensure holistic patient care.

Deliver evidence-based, patient-centric care that integrates advanced diabetes treatments and medication management within a value-based framework.

Empower patients and their families with knowledge about diabetes self-management, prevention, and the transformative impact of healthy lifestyle habits on long-term wellness.

Maintain thorough, up-to-date documentation in accordance with organizational and clinical standards, emphasizing patient outcomes and care quality.

Continuously engage in professional development and stay current with advances in lifestyle medicine and diabetes management to enhance care delivery.
Qualifications:

MD or DO degree, BC/BE in Family or Internal Medicine.

Licensed or eligible for licensure in Texas

Expertise in diabetes care and a passion for integrating lifestyle medicine to improve patient outcomes.

Strong communication and interpersonal skills, with a focus on building trusting, long-term relationships with patients.

Experience working within a value-based care model and proficiency in electronic health records (EHR) systems is preferred.
