Focus : Skilled Nursing, Long-Term Care, and Assisted Living Facilities, Primary Care Outpatient Clinic
Location : Various nursing and rehabilitation facilities in Rhode Island; locations are assigned based on population need, and candidate availability and distance.
Our primary care outpatient clinic is located in Cranston.
Overview : We are seeking a full-time Nurse Practitioner or Physician Assistant to join our team. The NP / PA will work independently and with providers and staff to manage and ensure comprehensive and continuous medical care of the patients under their care. This position is located in sub-acute and long-term care facilities, with the option of working in our PCMH-recognized primary care clinic.

Key responsibilities :
- Act as a primary healthcare provider within the facility, escalating to collaborative providers as needed, including : making clinical decisions, assessing and treating patients within the NP / PA’s scope of practice.
- Perform physical examinations and assessments to develop individualized care plans on a routine and as needed basis.
- Diagnose and treat chronic and acute conditions, including prescribing medications and treatments, advising and approving recommendations, and ordering and interpreting tests.
- Document patient encounters in accordance with the standards of State and Federal Regulations and the care facilities in a timely, accurate, and detailed manner.
- Collaborate with nursing and office staff, interdisciplinary teams, provider team, and external medical providers to comprehensively manage acute and chronic conditions.
- Communicate with patients and family members / representatives regarding care plans and progress on an as needed basis.
- Respond to calls and messages in a timely manner, including on a rotating on-call team and (if feasible) assisting unavailable team members.
- Maintain current licensure and certifications required for practice.
