Duties
- Conduct comprehensive patient assessments to identify needs and develop individualized care plans.
- Provide clinical counseling and behavioral therapy to clients facing mental health issues.
- Facilitate group therapy sessions to promote social interaction and support among clients.
- Offer guidance on aging issues and geriatrics, ensuring that elderly clients receive appropriate resources and care.
- Collaborate with healthcare professionals to coordinate home community care services for clients.
- Perform diagnostic evaluations to determine the appropriate interventions for clients.
- Maintain accurate records of client interactions, treatment plans, and progress notes in compliance with legal and ethical standards.
- Advocate for clients’ rights and access to necessary services within the community.
Required Qualifications:
Master's Degree Social Work from accredited university.
Coursework/Training Courses or continuing education in Evidence-Based Intervention.
1 year social work experience in a health care setting (outpatient/acute care or skilled nursing facility).
Preferred Qualifications:
