What are the responsibilities and job description for the Case Manager position at Medina County Board of Developmental Disabilities?
Summary: Responsible for the provision of the Service and Support Administrative functions as listed and defined in the ORC and OAC. Develop, plan, coordinate, monitor and evaluate services, programs, and supports which empower persons with developmental disabilities to live, work, and participate in the community. Ensure the rights of individuals and groups in accordance with federal, state and local statutes and regulations; facilitate the acquisition of needed or requested services; provide mentoring and technical assistance as needed; and address barriers to meet identified needs.
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
· Coordinate, facilitate, and monitor the development of individual service plans and author Life Plans. Activities may include, but are not limited to: observing, assessing, and consulting with individuals, guardians, families, and/or other involved persons to determine interest/needs, reviewing all documentation, interacting with other staff, summarizing/evaluating data, developing goals, drafting reports, scheduling/attending meetings as required.
· Facilitate the development of a circle of support which assists individuals in identifying and planning their life goals.
· Provide advocacy, training, coordination of resources and supports to the individuals, their circle of support and other involved parties as necessary.
· Promote the involvement of agencies/individuals to arrange services that significantly impact the life of the individual while eliminating duplication. Assist the individual in obtaining appropriate agency or community-based services.
· Attend appointments and intake meetings as appropriate.
· Monitor services to assure that they are being delivered according to the plan in accordance with established time frames, standards and outcomes.
· Maintain records and reports in a timely and accurate manner, consistent with accreditation and regulatory standards or contracted agreement.
· Facilitates the development and monitoring of crisis intervention activities in conjunction with other involved individuals. Ensures that necessary services are available on a 24 hour basis and crises are responded to within a timely manner.
· May perform follow-up activities as result of an unusual incident/major unusual incident. Facilitate revision to individual plans as a result of patterns/trends identified in unusual incident/major unusual incident reports.
· Maintain contact with individuals and their circle of support, and frequent contact with family members, service providers, and employers, as appropriate.
· May arrange for transportation for individuals or may provide transportation as a last resort.
· Assess and evaluate cases and prepare and implement treatment and service plans.
· Maintain working relationships with staff of other agencies and facilities, and acts as liaison between consumers and agency.
· Provides consultation to staff on specific cases.
· Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required.

Pay: $45,000.00 - $60,000.00 per year
