Job Description
Responsibilities / Job Duties :
As a Case Manager for DC Medicaid, you will support enrollees, many of whom have behavioral health issues and experience poor social determinants of health. You will be responsible for conducting needs assessments, assigning acuity levels to enrollees, and determining the frequency of contact based on their individual needs.
- Acuity Levels
- Level 1
Well-managed enrollees with minimal Medicaid use.
Moderate Medicaid use, needing more frequent attention.
High utilizers with complex needs, requiring more frequent touchpoints.
Your role will include providing case management services, conducting home visits, developing care plans, and collaborating with primary care providers to ensure the best outcomes for enrollees.
Typical Case Management :
What does the caseload look like?
75 patients across levels 1, 2, and 3.
60 patients with complex needs (level 3 acuity).
