Job Title : Bilingual (Spanish) Case Manager
Location : Suffolk County
Pay : $18-$22
Overview :
As a Case Manager ( Must be bilingual in Spanish) you will be an integral member of an interdisciplinary team, providing care coordination for a caseload of mentally ill adults, many of whom have medical comorbidities and / or co-occurring substance abuse disorders. You will advocate for and support clients by engaging with community agencies, healthcare providers, and other stakeholders to ensure access to services that promote wellness, self-management, and reduce emergency room visits and hospitalizations.
Key Responsibilities :
- Provide clinical support to the team through consultation, education, and information on conditions, interventions, and resources, ensuring focus on best practices and outcome-driven approaches.
- Conduct initial and ongoing assessments of clients to document strengths, needs, goals, and available resources.
- Collaborate with hospital care teams during admissions and discharges to ensure seamless discharge planning and arrange for appropriate outpatient follow-up care.
- Assist clients with applications for benefits, including SNAP, SSI / SSD, HEAP, and other necessary support programs.
- Address social determinants of health, such as food insecurity, transportation, and housing issues.
- Provide face-to-face outreach, engagement, and service planning in the field.
- Act as a liaison to community services, including medical, behavioral, residential, entitlement programs, and any other needed services outlined in the interdisciplinary care plan.
- Provide crisis intervention and follow-up as needed.
- Maintain accurate and timely documentation of all client interactions, records, and related reports, adhering to organizational policies and procedures.
- Perform other duties as assigned that are reasonably related to the role.
