Medix is looking to hire a Community Outreach Worker in Solano County to support and represent the CalAIM program, advancing the mission, vision, and philosophy of the agency. Responsibilities : Conduct outreach to eligible community members in a variety of settings and provide insight of the program. Assist candidates with the enrollment process and educate them on the benefits of doing so. Establish and maintain a network with local clinics, hospitals, and other medical facilities to host events for member enrollment. Provide excellent customer service during patient interactions. Maintain thorough documentation of enrollments and patient information. Requirements / Experience : Community Health Worker certification (highly preferred) Personal vehicle to commute to different locations and set up enrollment events Driver's License and Auto insurance Self-starter and goal-driven personality to operate with minimal direct supervision BA degree in Sociology, Psychology, or Public Health Schedule : Monday- Friday between the hours of 8 : 00 - 3 : 00 pm 6-hour work days 30 hours a week Pay range : $25-28 / hr BOE Mileage reimbursement For California Applicants : We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO) , and the California Fair Chance Act (CFCA).
