Exciting Opportunity for a Nurse Practitioner!
A Healthcare Management Group is seeking a Primary Care Nurse Practitioner to provide comprehensive nursing and medical care to geriatric patients in a Nursing home located in Hamden, CT. The individual must adhere to organizational policies and standards while demonstrating adequate knowledge, technical skills, and judgment.
Responsibilities include:
- Managing all consults related to Primary Care
- Conducting history and physical exams
- Reviewing medications
- Prescribing medication/drugs
- Following up with patients who have acute issues and establishing follow-up plans
- Reaching informed diagnoses based on medical knowledge and patient's medical history
Schedule:
- Monday-Friday
- 8-hour shifts
- No weekend work
