Our client, a large Physician Organization , is looking for an Adult RN Care Manager . The position is full-time with a great compensation and benefits package.
Salary : $75,000-85,000 / yr, depending on previous experience.
Location : Oakland County, MI. Candidate will work out of their home, visit the practices, have occasional office meetings, and must have a secure environment at home.

Job Description :
The Adult RN Care Manager will help the primary care physician network by providing service to patients with chronic conditions who are at risk for disease progression, complications, cognitive and functional decline. The RN Care Manager develops and implements an individualized care plan designed to promote the patient, family / caregiver’s understanding, and
management of their condition, optimize quality of life, and improve clinical outcomes.
This position provides service in the primary care practices, telephonic outreach, and telemedicine.
Responsibilities & duties :
- Engage moderate / high-risk patients through outreach, telemedicine, and occasional visits to the practice.
- Conduct comprehensive assessments (practice, telemedicine, or phone) to identify patient needs, goals, and risks.
- Develop and monitor individualized care plans with the care team to improve outcomes and satisfaction.
- Support and establish patient self-management goals.
- Educate patients on chronic condition management to prevent disease progression and promote healthy behaviors.
- Coordinate care transitions and monitor high-risk patients post-discharge to prevent readmissions.
- Facilitate discussions on advance directives and palliative care.
- Connect patients to resources, referrals, and community services.
- Participate in Care Management meetings, conferences, and huddles.
- Maintain accurate and timely documentation and billing of care encounters.
- Collaborate with practices to improve care management processes.
- Maintain confidentiality and follow relevant policies.
- Participate in committees and perform duties as assigned by management.
- Evaluate and close gaps in patient health services (HEDIS metrics).
- Maintain education and BLS requirements for Complex Care Management.
