Our client is seeking a Care Review Clinician II to use clinical expertise in ensuring patients receive the right level of care at the right time .
Key Responsibilities :
- Review and coordinate high-level care transfers for complex patients.
- Assist the Complex Discharge Unit to ensure seamless member transitions .
- Collaborate with hospitals, higher-level care facilities, and providers .
- Utilize MCG inpatient criteria to determine appropriate transfer needs.
- Work with health plan networks to ensure members are placed in PAR hospitals whenever possible.
