Position : Transition of Care (ToC) Coach
Remote – Candidates must be located in one of the following states : AZ, FL, GA, ID, IA, KY, MI, NE, NM, NY (outside greater NYC), OH, TX, UT, WA (outside greater Seattle), WI
Pay Rate : $50.05 hourly (Benefits Available)
Schedule : Monday - Friday, 8 : 30 AM – 5 : 00 PM EST
Requirements : In NY State / PN with an unrestricted NY state license
About the Role :
As a Transition of Care (ToC) Coach , you will play a vital role in ensuring safe and effective care transitions for members moving from acute care settings to lower levels of care, such as skilled nursing facilities or private homes. Your primary focus will be on reducing readmission risks by providing comprehensive member education, coordinating care, and connecting members with the necessary resources.
This role is 100% remote , supporting hospitalization and transitions of care for members on designated Care Management teams. There is no fieldwork or specific caseload assignment — you will collaborate with hospital discharge planners, primary care providers, specialists, and other healthcare professionals to facilitate seamless transitions.
Key Responsibilities :
Guide members through transitions from inpatient settings to lower levels of care, ensuring a safe and effective discharge process
Collaborate with healthcare providers, skilled nursing facilities, clinical pharmacists, behavioral health providers, and community-based organizations to coordinate post-discharge care
Conduct timely assessments and interventions to prevent unnecessary readmissions
Educate and support members in self-managing chronic conditions using motivational interviewing techniques
Assist members in addressing barriers to care by providing care coordination and resource navigation
Participate in Multidisciplinary Team (MDT) meetings to ensure holistic and patient-centered care
Qualifications :
In NY State / LPN with an unrestricted NY State license (Required)
Experience working with vulnerable populations and understanding their healthcare needs
Strong background in care coordination, patient education, and chronic disease management
Familiarity with motivational interviewing techniques and ability to empower patients toward self-management
Ability to work in a highly collaborative, team-oriented remote environment
Technical Requirements :
Must have dual monitors and a docking station for optimal workflow efficiency
