Job Description

We are seeking a dedicated Home Health Occupational Therapist to join our healthcare team. The ideal candidate will provide quality care and support to patients in need of occupational therapy services.

Duties :

Conduct patient assessments to determine individual needs and develop customized treatment plans
Implement occupational therapy interventions to enhance patients' independence and quality of life
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Monitor and document patient progress and adjust treatment plans as needed

Qualifications :

Bachelor's or Master's degree in Occupational Therapy from an accredited program

Valid state licensure as an Occupational Therapist

Strong knowledge of anatomy, medical terminology, and medical imaging

Experience working with diverse patient populations with a focus on geriatrics

Proficiency in patient care, case management, and occupational health practices
