We are seeking an Advanced Practice Practitioner for our Post-Acute team in the Newark or Zanesville area who shares our uncommon passion for excellence and commitment to put team before self in pursuit of making patient care the best it can be.
Status: Full-time
Qualifications:
- Graduate of an accredited physician assistant or nurse practitioner program (Family Nurse Practitioner or Adult-Gero Nurse Practitioner)
- Current national certification as physician assistant or nurse practitioner.
- Eligible for privileges to post-acute facilities
Responsibilities:
- Evaluates new and existing patients by performing a history and physical and interpreting laboratory and diagnostic studies for complex acute and chronically ill patients on the MedOne Service. Recognizes deviations from normal physical exam and laboratory/diagnostic studies.
- Works in collaboration (nurse practitioners) or under the supervision (physician assistants) with a physician in developing treatment plans for health problems and follow-up.
- Orders and interprets diagnostic and laboratory tests related to the patient’s needs.
- Prescribes appropriate pharmacologic medications in accordance with prescriptive authority regulations. Prescribes appropriate non-pharmacologic treatment modalities.
- Monitors the effectiveness of interventions, makes adjustments to care plans, and communicates to the collaborating/supervising physician as needed.
- Collaborates with multidisciplinary team members by making appropriate referrals and consultations.
- Facilitates the patient’s transition within and between health care settings such as admitting, transferring, and discharging patients.
- Communicates treatment plan between patient, family, and the medical team.
- Maintains patient and family confidentiality, privacy, and patient rights.
- Participates in case conferences and team meetings to discuss patient progress, discharge planning and care transitions.
- Provides other care or services as needed within scope of practice to facilitate the care of patients.
- Attends MedOne meetings on a regular basis.
- Documents in the patient record as appropriate according to MedOne and institution guidelines.
- Adheres to credentialing and privileging process in order to maintain clinical privileges.
- Bills or logs encounters as directed.
- Acts as a role model and resource to other advance practice clinicians and nursing staff.

The credentialing process can typically take 90-120 days from the date the credentialing applications are submitted for processing. Please keep in mind, the credentialing timeframe does not start until the applications have been submitted for processing.
Credentialing will need to obtain your malpractice claims history from the past 5 years. Please be aware your current employer may be notified you are accepting another position.
