The physical therapy care coordinator will be responsible for assessing patients' medical conditions, functional capabilities, limitations, and restrictions, and establishing treatment plans under the direction of a physician. This includes developing individualized plans of care, implementing interventions, and documenting patient progress.
Education and Licensure
To be successful in this role, applicants must hold a Doctorate degree in Physical Therapy or a Bachelor's degree in Physical Therapy with a passing score on the NPTE. Additionally, they must have graduated from an accredited school and possess a current state license as a Registered Physical Therapist or be actively pursuing licensure.
