Comprehensive Care : Conduct in-person assessments and coordinate individual care plans. Clinical Oversight : Manage admissions, discharges, and ongoing care while guiding the team. Integrated Role : Provide primary care, collaborate on discharge planning, and cover rotating call. Qualifications : Board Certified (Internal Medicine / Family Practice, Geriatrics preferred), active license, CDS, DEA, valid driver's license, and current vehicle insurance.
