About the Facility: Our client, a reputable healthcare provider in Hempstead, is seeking a Spanish-speaking Social Worker to join their team for home care patient intake. This role involves evaluating home care patients' needs and ensuring they receive the proper care and support. The social worker will work in the office in Hempstead and communicate directly with patients to provide comprehensive care.
Position Overview: As the Social Worker for home care intake, you will be responsible for assessing new patients, establishing care plans, and ensuring they receive the necessary services. Fluency in Spanish is required to effectively communicate with the diverse patient population. You will need to have one of the following certifications: MHC, LMHC, MSW, or LMSW.
Key Responsibilities:
- Conduct intake assessments for home care patients, determining their needs and appropriate services.
- Develop individualized care plans for patients, ensuring they receive the necessary support and services.
