Salary : $40 - $45 per hour

Schedule : Monday to Thursday, 9 : 00 AM - 5 : 00 PM, Friday 9 : 00 AM - 3 : 00 PM

About the Facility : Our client, a primary care center in Hempstead, is seeking a compassionate and experienced Spanish-speaking LPN to join their team. This position is focused on case management, where you will follow up with patients post-appointment to ensure they have received their medications, are feeling better, and address any remaining concerns they may have.

Position Overview : As the Spanish-speaking LPN in this role, you will primarily manage the continuity of care for patients by providing follow-up care after their visits to the primary care clinic. You will contact patients to ensure they are recovering as expected, assist them with medication management, and confirm whether they need further assistance or follow-up with the physician.

Key Responsibilities :

• Contact patients after their appointments to check on their well-being and recovery progress.
• Confirm that patients have received their prescribed medications and are taking them as directed.
• Address any concerns or issues patients may have regarding their care and medications.
• If patients feel they still need to see the doctor, assist in scheduling follow-up appointments.
• Document interactions and feedback in the patient's medical records.
• Collaborate with physicians and the medical team to ensure patients' needs are met.
