A well-established community health organization in the Plymouth, MA area is seeking a Family Nurse Practitioner to join its primary care team. This full-time, outpatient position offers the opportunity to provide comprehensive care to a diverse patient population, including routine wellness visits, chronic disease management, and acute care. The ideal candidate will have at least 2 years of experience, the ability to work independently, and a commitment to serving underserved communities.
This role offers a collaborative, team-based environment, where providers work alongside RNs, MAs, and interpreters to ensure high-quality, patient-centered care. The schedule is Monday through Friday, with one evening shift per week and a rotating Saturday schedule based on clinic needs. The clinic utilizes Epic EMR and has a strong support system, including on-site behavioral health and care coordination services.
