Job Title : Occupational Therapist - Home Health

Job Specialty : Home Health Occupational Therapy

Job Description :

Provide occupational therapy in a home health setting for adults and geriatrics.

Perform assessments, discharges, evaluations, OASIS, plan of care, and reassessments.

Manage a daily patient load equivalent to 6 points per day, meeting productivity standards of 30 points per week.

Proven skills in various rehabilitation programs, including amputee, arthritis, CVA rehabilitation, dementia / Alzheimer's, hand injury, traumatic brain injury, and more.

Conduct home assessments, pre-discharge planning, and wheelchair evaluation & positioning.

Educate patients and families, focusing on safety, injury prevention, and infection prevention.

Familiarity with adaptive equipment and community re-entry strategies.

Complete standardized assessments, as well as lift and transfer patients as necessary.

Wear appropriate attire as specified (Navy pants, white or navy top).

Preferred Skills :

Experience in National Patient Safety Goals, adult outpatient, adult rehabilitation facility, long-term acute care (LTAC), pediatric rehabilitation, skilled nursing (SNF), coma management, and joint mobilization.

Proficiency in working with knee prosthetics and dynamic, functional, and static splints.

Capabilities in crisis intervention, substance abuse treatment, and cognitive / perceptual assessment.
